Getting prescriptions by mail
 

If you have a prescription from your doctor:

  1. Register now and access Forms and Cards from the left navigation.
     
  2. Download and print a mail order form by clicking the "print mail order form" link on the Forms page.
     
  3. Mail the completed form and your prescription slip(s) to the address indicated.

Click here to learn more or contact a customer service representative by calling the number on your pharmacy/health plan benefit card. Many times, a representative can help you transfer your prescriptions to the Medco pharmacy.

If you are going to see your doctor:
Before you go to your doctor, download the physician fax form* and ask him or her to fill it out and fax it to the number indicated on the form.

Please note: Only your physician may fax the completed form to us.

*The physician fax form is an Adobe® Portable Document Format (PDF) file. You will need to download a free copy of Adobe® Reader® if you do not have it already.
